Get A New York Medical Marijuana Card - NY [5 Steps]
The first step in the medical marijuana application process in New York is to work with a medical practitioner who can certify that you meet the criteria for a qualifying condition. The practitioner can be a physician, physician’s assistant, or nurse practitioner that is licensed to practice in New York and registered with the state’s medical marijuana program.

Step 2: Get Your Patient Certification
Once you and your medical practitioner have discussed your qualifying conditions, they will complete a patient certification and upload it to the New York Department of Health for approval. You will also receive a printed and signed copy of the certification so that you can complete your portion of the online application.
Step 3: Complete Your Medical Marijuana Application
After your practitioner submits your patient certificate, you’ll need to create an account on NY.gov and complete your application online.
The online application form will request that you submit the practitioner certification form and proof of residency. There are a number of ways to prove permanent or temporary residency in the state of New York:
- ID number from your New York State Driver's License or New York State Non-Driver ID Card - Photocopy of a government-issued ID card that contains your New York State address - Photocopy of a utility bill with a New York State address that shows proof of payment for at least two months - Photocopy of a current mortgage or lease showing residency in New York State
Patients receiving treatment in New York for a qualifying medical condition can prove temporary residency by submitting a lease, utility bill, or hospital bill.
Step 4: Get Your Card
Once your application is submitted and approved, you’ll receive a digital medical cannabis card to print and use for up to 30 days. Your permanent, hard copy will arrive in the mail in around 7-10 days.
Medical cannabis patients in New York are allowed to designate up to two caregivers who can purchase and administer products on their behalf. Caregivers will also need to register. Obtain their own medical marijuana card.
